Register with the Practice & Boundaries

We can only accept new patients who live in our practice area (within 4 miles from the practice).

You will be asked by one of Reception Team for details of your name, address, phone number.

This information will then be passed onto the New Registration Department to check your address and we will be back in touch with you in due course to let you know if you are in our area or not.

If you are not in our area you can go on the NHS Choices Website and look up to see where your nearest GP Surgery is.

If you are in our catchment area we will ask you to collect the forms from Reception, this will include a Registration form (GSM1) and the Practice New Patient Health Form.

When registering with Abbots Bromley Surgery you will need to bring proof of address (Utility Bill) plus a Driving Licence or Passport, and show the Receptionist.

When you register, we will ask you to leave your forms with us for 7 – 10 days, to ensure we have your recent health history from your previous GP.

Temporary Residents

If you are ill while away from home or if you are not registered with a doctor but need to see one you can receive emergency treatment from the local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.

To register as a temporary patient simply contact the local practice you wish to use. Practices do not have to accept you as a temporary patient although they do have an obligation to offer emergency treatment. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
